Why Didn’t My Embryo Implant?
One of the most difficult moments during an IVF journey is hearing: “The embryo didn’t implant.” It’s natural to ask yourself: Was something wrong with the embryo? Was there something wrong with my uterus? Should I have more testing? Is there anything I can do differently before my next frozen embryo transfer (FET)?
The honest answer is that implantation is one of the most complex stages of reproduction. Even a high quality embryo does not automatically result in a pregnancy. Successful implantation depends on healthy communication between the embryo and the uterine lining during a brief period known as the window of implantation. The Three Weeks Before Embryo Transfer: One of the Most Important Opportunities to Support Implantation
Many patients think preparing for embryo transfer begins on transfer day. We believe it begins weeks earlier. The three weeks leading up to embryo transfer provide one of the best opportunities to prepare the uterine environment for implantation. During this time, the endometrium is developing, hormones are preparing the uterus to receive an embryo, and many of the biological systems involved in implantation are becoming active.
Although the embryo is critically important, implantation also depends on the environment it enters. A healthy uterine environment includes much more than the thickness of the uterine lining. Blood flow, hormonal balance, healthy inflammation, the vaginal and uterine microbiome, metabolic health, mitochondrial function, nervous system regulation, and overall health all contribute to uterine receptivity.
Research suggests acupuncture performed before and after embryo transfer on transfer day may improve IVF outcomes. During the three weeks leading up to embryo transfer, acupuncture is used to help prepare the uterine environment for implantation.
Large systematic reviews and meta analyses have also reported improvements in implantation, clinical pregnancy, and live birth rates when a series of acupuncture treatments are incorporated leading up to transfer day. A series of acupuncture treatments leading up to embryo transfer provides greater benefit than just a treatment on transfer day alone.

Whenever possible, we recommend a series of IVF acupuncture treatments during the three weeks leading up to embryo transfer, followed by treatment immediately before and immediately after embryo transfer on transfer day.
Professional Photobiomodulation (PBM) / Low Level Laser Therapy (LLLT)
Professional photobiomodulation, also known as PBM, low level laser therapy (LLLT), laser acupuncture, and red light therapy, is frequently combined with acupuncture as part of our fertility program. Research suggests PBM may support mitochondrial function, cellular energy production, circulation, healthy gut microbiome and inflammation, tissue repair, and reproductive health. Research has also demonstrated that PBM may help activate the parasympathetic nervous system, supporting the body’s natural relaxation response.
At Acubalance Vancouver, we combine professional PBM with acupuncture throughout the three weeks leading up to embryo transfer because each therapy supports fertility through different biological mechanisms. Together, they may provide broader support for implantation than either therapy alone. We use professional grade PBM systems and fertility specific treatment protocols. Our Embryo Transfer Acupuncture Protocol
For patients having embryo transfer at Olive Fertility Centre, Acubalance practitioners provide two acupuncture treatments on transfer day, one immediately before embryo transfer and one immediately afterward onsite at Olive. Since 2002, we have proudly provided onsite IVF/acupuncture on transfer day. Acubalance Vancouver goes onsite to Olive Fertility Centre allowing patients to receive treatment without the added stress of travelling between clinics.
For patients receiving care at PCRM, Twig Fertility, Grace Fertility, or other IVF clinics, we provide acupuncture combined with laser acupuncture at Acubalance Vancouver. We generally recommend treatment the day before embryo transfer, followed by another treatment within 48 hours after embryo transfer for both fresh and frozen embryo transfers.

Should I Have More Testing Before My Next Embryo Transfer?
One of the most common questions after an unsuccessful embryo transfer is: “Should I have more testing before trying again?” The answer depends on your individual fertility history.
Some patients benefit from additional testing before their next embryo transfer. Others do not. The goal isn’t to order every available test. It’s to identify which investigations are most likely to provide information that could influence your treatment plan. At Acubalance Vancouver, we begin by reviewing your fertility history, previous IVF cycles, embryo quality, uterine lining, laboratory results, medical history, and any previous investigations. Based on that review, we may recommend discussing additional testing with your reproductive endocrinologist or arranging complementary testing when appropriate.

Assessment of the Uterine Cavity
Your fertility specialist may recommend investigations such as saline sonography, hysteroscopy, or other imaging to evaluate for uterine polyps, fibroids, adhesions, scar tissue, or congenital uterine abnormalities that could interfere with implantation.
Endometrial Assessment
When clinically appropriate, your fertility specialist may recommend an endometrial biopsy or other investigations to evaluate the uterine lining, including assessment for chronic endometritis, endometrial receptivity, or other conditions that may affect implantation.
Microbiome Testing
Growing research suggests both the uterine and vaginal microbiomes may influence implantation and reproductive health. When clinically appropriate, your fertility specialist may recommend testing the uterine microbiome as part of your IVF investigation. At Acubalance, we may also recommend vaginal microbiome testing, such as the Evvy Vaginal Health Test, as well as gut microbiome testing, since all three microbial environments may influence inflammation, immune regulation, hormone metabolism, and reproductive health.
When appropriate, we combine testing with personalized nutrition, targeted probiotics, lifestyle strategies, and other therapies designed to support a healthier microbial environment before embryo transfer.
Sperm DNA Fragmentation
A normal semen analysis does not always tell the whole story. In some cases, sperm DNA fragmentation testing may provide additional information about sperm quality that can influence fertilization, embryo development, implantation, and miscarriage risk.
If you have experienced unsuccessful embryo transfers and do not have any embryos remaining, this may be an appropriate time to investigate sperm DNA fragmentation before creating new embryos. Optimizing sperm quality before another egg retrieval, together with improving the ovarian environment during the final 100 days before retrieval, may improve the chances of creating healthier embryos for future transfer. Hormonal and Metabolic Assessment
Depending on your history, additional blood work may be appropriate to evaluate thyroid function, blood sugar regulation, iron status, vitamin and nutrient levels, inflammation, hormone balance, or other metabolic factors that may influence reproductive health.
The purpose of additional testing is not simply to gather more information. The goal is to identify findings that may change your treatment plan and create new opportunities to improve your chances of implantation. Not every patient needs more testing. Not every test is appropriate for every patient.

Does laser acupuncture or photobiomodulation help implantation?
Photobiomodulation, also known as PBM, low-level laser therapy, LLLT, red light therapy, is being studied as a supportive therapy for implantation, uterine receptivity, and frozen embryo transfer outcomes.
PBM uses specific wavelengths of red and near-infrared light to stimulate cellular activity. In fertility care, the goal is to support mitochondrial function, circulation, tissue repair, lymphatic flow, and healthy inflammatory signaling. This may matter for implantation because the uterus needs adequate blood flow, cellular energy, immune balance, and a receptive endometrium for the embryo to attach and develop.
At Acubalance Vancouver, we often combine professional PBM with acupuncture in the weeks before FET and on transfer day because each therapy supports fertility through different mechanisms. Acupuncture may help regulate the nervous system, improve uterine blood flow, and support hormonal balance, while PBM may support cellular energy, healthy inflammation, blood flow and tissue repair.
Research is still emerging, but several studies are promising. A 2024 randomized clinical trial found higher biochemical pregnancy rates (46.7% vs. 33.3%) and clinical pregnancy rates (33.3% vs. 20%) in women with recurrent implantation failure receiving LLLT before frozen embryo transfer. Research also suggests that laser pretreatment may improve endometrial preparation for frozen embryo transfer in women with recurrent implantation failure, possibly by improving microcirculation and supporting growth factors and cytokines involved in receptivity.
For patients preparing for FET, Acubalance commonly recommends PBM and acupuncture in the weeks leading up to transfer, often 2–3 sessions per week depending on the patient’s history, lining response, age, inflammation, endometriosis, PCOS, or previous implantation failure. So, does PBM help implantation? It may help support the conditions implantation depends on, especially uterine blood flow, endometrial receptivity, cellular energy, and inflammation regulation. It should be viewed as an adjunctive fertility support as more research is needed.
